London: Inspectors submitted a report to ministers on Monday into children's welfare services at the north London borough where Baby P died.
Children's Secretary Ed Balls ordered the emergency review at Haringey Council last month after the conviction of three people over the 17-month-old boy's death.
The toddler died after suffering more than 40 injuries during a campaign of abuse at his home, despite being on Haringey's "at risk" register and having had 60 visits from care workers.
It was the second serious child welfare tragedy in Haringey following the murder of 8-year-old Victoria Climbie by her guardians in 2000.
"Inspectors have given the report to the Secretary of State and ministers are considering what action to take," said a spokeswoman for the Department for Children, Schools and Families.
Agencies failed
Schools inspectorate Ofsted, the Commission for Healthcare Audit and Inspection and the Chief Inspector of Constabulary have conducted the review.
Balls has already said that agencies had failed to follow proper procedures before the death of Baby P.
